Unemployment Form for Employees

The State of Pennsylvania has passed ACT 32, which requires EVERY employer to withhold local taxes for EVERY employee starting in January, 2012. It also reorganizes the state into new taxing districts. All the taxes that you collect from your employees will be remitted to one taxing authority. Allegheny County has been divided into four districts which will be serviced by either Keystone Collections or Jordan Tax Service.

In order to properly allocate the taxes that are collected, each employee is required to complete a  Certificate of Residency. This form is to be kept with your employee records along with their  W-4 and  I-9. You can download copies from here. We will also be sending the forms with your third quarter quarterly reports.
